Faith Formation Registration

Thank you for your interest in you and/or your child growing in their Catholic faith! Know that we here at St. Anastasia are here to support you on your faith journey as you come to grow deeper in your relationship with God and live out His call to holiness.

In order to ensure we best serve your needs, please review the following information below. If you still have any questions, please contact the Parish Office at 772-461-2233.

All those who wish to register for our faith formation program must be a registered parishioner at St. Anastasia. If you are not registered, please fill out our New Parishioner Registration Form.

Do you seek your child to receive faith formation and do not attend St. Anastasia Catholic school? Please go to Sunday Faith Formation

Do you seek your child to receive the Sacraments of First Reconciliation and First Holy Communion and attend St. Anastasia Catholic school? Please go to 2nd Grade Sacraments

Do you seek your child to receive the Sacraments of First Reconciliation and First Holy Communion and do not attend St. Anastasia Catholic school? Please go to Sunday Faith Formation

Do you seek your child in 8th Grade to prepare to receive the Sacrament of Confirmation and do not attend St. Anastasia Catholic school? Please go to Sunday Faith Formation

Do you seek your child in 9th Grade to prepare to receive the Sacrament of Confirmation? Please go to Confirmation

Do you seek your child to receive any of the Sacraments of Initiation (Baptism, Confirmation, First Holy Communion) and have not received at the “typical” age (2nd grade for First Holy Communion and 9th grade for Confirmation)? Please go to RCIC/RCIT

Are you over the age of 18 and wish to receive any of the Sacraments of Initiation (Baptism, Confirmation, First Holy Communion)? Please go to RCIA

In order to complete registration, please come to the St. Anastasia Parish Office (M-F 8am to 4pm) and bring the following:​​​​​

• Form of payment: credit card or check made payable to "St. Anastasia Catholic Church" and pay registration fee (see specific page for registration fee);

• Sacramental certificates of you or your child: Baptism, First Holy Communion (if applicable
